Job Summary
Supporting patients and caregivers navigating dementia-related care, the full-time Care Coordinator for Dementia Care will manage outreach, follow-up, documentation, and cross-team communication to ensure smooth care pathways while working remotely.
Key Responsibilities
- Conduct outreach to patients and caregivers to support care coordination needs and manage follow-up across a dynamic caseload
- Accurately document all outreach attempts and patient interactions in electronic health records and tracking systems
- Communicate patient updates clearly to nursing, intake, support, and leadership teams, flagging any emerging issues or concerns
Required Qualifications
- Minimum 3 years of experience in care coordination, case management, or patient navigation
- Experience working with older adults and/or patients with dementia and their caregivers
- Strong ability to take initiative and manage multiple patients and priorities
- Minimum of 1 year experience working in a remote environment
- Excellent organizational and documentation skills
