Key Responsibilities

• Facilitates a team approach with the Interdisciplinary Care Plan team to ensure quality care and services
• Conducts in-depth health risk and comprehensive needs assessments across various parameters
• Provides care coordination for members with chronic or complex conditions, implementing and monitoring strategies for improved health outcomes

Required Qualifications

• Master's degree with 4 years of experience, Bachelor's degree with 8 years, or Associate's degree with 9 years of experience; 12 years of experience may substitute for education
• Proficiency in Microsoft Word, Excel, and Outlook
• Experience in analyzing trends using decision support systems
• Business management skills, including cost/benefit analysis and negotiation
• Knowledge of referral coordination to community and private/public resources
