Job Summary

A company is looking for a Care Coordinator IV.

Key Responsibilities

• Facilitates a team approach with the Interdisciplinary Care Plan team to ensure appropriate interventions and quality care delivery
• Conducts in-depth health risk assessments and comprehensive needs assessments covering various parameters
• Provides care coordination for members with chronic or complex conditions, implementing and monitoring strategies to improve health outcomes

Qualifications

• Master's degree with 4 years of experience, Bachelor's degree with 8 years of experience, or Associate's degree with 9 years of experience; 12 years of experience may substitute for education requirements
• Proficiency in Microsoft Word, Excel, and Outlook is required
• Experience in analyzing trends based on decision support systems
• Business management skills including cost/benefit analysis and negotiation
• Knowledge of referral coordination to community and private/public resources
