Job Summary

Facilitating a team approach to care coordination, the full-time Care Coordinator IV - NM Licensed will manage interdisciplinary care plans, conduct comprehensive health assessments, and provide support for members with chronic or complex conditions in a remote work environment.

Key responsibilities

• Facilitates collaboration among the interdisciplinary care plan team to ensure effective and quality care delivery
• Conducts in-depth health risk assessments and comprehensive needs assessments across various parameters
• Coordinates care for members with chronic or complex conditions, implementing and monitoring strategies to improve health outcomes

Required qualifications

• Master's degree with 4 years of experience, Bachelor's degree with 8 years of experience, or Associate's degree with 9 years of experience; 12 years of experience may substitute for education requirements
• Proficiency in Microsoft Word, Excel, and Outlook
• Experience in analyzing trends using decision support systems
• Business management skills including cost/benefit analysis and negotiation
• Knowledge of referral coordination to community and private/public resources
