Care Coordinator - New Jersey

Job Summary

To support a growing pediatric practice, the part-time Care Coordinator - New Jersey will provide clinical and clerical support, manage patient communications, and ensure quality care in a remote environment.

Key responsibilities

• Manage incoming and outgoing patient communications, including clinical inquiries and lab result notifications
• Ensure pre-visit preparations are completed and follow up with patients regarding appointments and care transitions
• Assist the care team in completing necessary forms and reports to manage patient health needs effectively

Required qualifications

• High-school level education or equivalent
• 2-4 years of healthcare experience in outpatient clinics, preferably in specialty/multispecialty settings
• Spanish fluency preferred
