Required Qualifications, Training, and Education

• Bachelor's degree
• Minimum of one year in current position for internal applicants
• Healthcare and/or community-based experience with diverse populations preferred
• Bilingual (English/Spanish) preferred
• Proficiency in MS Office and ability to learn care coordination applications
