Care Coordinator - New York Licensed

Job Summary

To support a diverse patient population, the full-time Care Coordinator - New York Licensed will coordinate care, provide education and guidance to patients, and collaborate with care teams in a hybrid work environment that includes on-site visits and remote work.

Key responsibilities

• Coordinate care and develop comprehensive care plans in collaboration with the patient's care team
• Provide health education related to symptom management and preventive care while advocating for necessary services
• Utilize behavioral change modalities to coach patients and caregivers on lifestyle modifications

Required qualifications

• Bachelor's degree
• Experience in healthcare or community-based settings with diverse populations and complex chronic illnesses preferred
• Proficiency in MS Office and ability to learn care coordination applications
• Commitment to engaging patients in community-based care models
• Bilingual (English/Spanish) preferred
