Care Coordinator - New York Licensed

Job Summary

To support comprehensive care management initiatives, the full-time Care Coordinator - New York Licensed will coordinate patient care, provide education and support, and collaborate with care teams in a hybrid work environment that includes on-site visits and remote work.

Key responsibilities

• Coordinate care and develop comprehensive care plans in collaboration with the patient's care team
• Provide health education and advocate for patients to ensure they receive necessary services
• Utilize behavioral change modalities to coach patients and family caregivers on lifestyle modifications

Required qualifications

• Bachelor's degree
• Healthcare and/or community-based experience with diverse populations and complex chronic illnesses preferred
• Proficiency in MS Office and ability to learn care coordination applications
• Minimum of one year in current position
• Bilingual (English/Spanish) preferred
