Care Coordinator - Ohio Licensed

Job Summary

To support youth with complex needs and their families, the full-time Care Coordinator - Ohio Licensed will engage with clients, conduct assessments, and develop care plans while working remotely from Wood County.

Key responsibilities

• Effectively engages children/youth with significant behavioral health needs and their family/caregivers for an assigned caseload
• Conducts comprehensive CANS assessments and facilitates the development of individualized Child and Family-Centered Care Plans
• Facilitates Child and Family Team meetings to ensure the child and family have a voice in their care plan and access to necessary resources

Required qualifications

• At least 3 years of experience in community-based services for children and youth, or relevant education with reduced experience
• Background in family systems, community resources, case management, or related fields
• Culturally competent with experience managing complex cases involving children and families
• Ability to complete high-fidelity wraparound training within specified timeframes
• Valid Driver's license and acceptable driving record, with the ability to meet internet service requirements for remote work
