Job Summary

Seeking a full-time remote Care Coordinator - Ohio Licensed, the position focuses on comprehensive care coordination for youth with complex needs and their families, requiring effective engagement, resource linkage, and individualized care planning.

Key responsibilities

• Engage children/youth with significant behavioral health needs and their families for an assigned caseload
• Conduct comprehensive assessments and facilitate the development of individualized care plans
• Monitor service delivery and address challenges, ensuring ongoing engagement and documentation

Required qualifications

• At least 3 years of experience in community-based services for children and youth, or relevant education and experience combination
• Background in family systems, case management, or child development
• Experience working with children and families facing significant emotional disturbances or trauma
• Must complete high-fidelity wraparound training within a specified timeframe
• Valid Driver's license and acceptable driving record required
