Care Coordinator, Remote

Job Summary

To support patients in the Pacific Time Zone, the remote Care Coordinator will manage patient engagement, care coordination, and resource navigation while collaborating with an interdisciplinary team to enhance health outcomes for individuals living with kidney disease.

Key responsibilities

• Perform telephonic outreach to enroll and engage patients, serving as the primary contact for enrolled individuals
• Develop and implement care plans in collaboration with Nurse Care Managers, ensuring cohesive support throughout the patient's healthcare journey
• Conduct assessments to identify social determinants of health and connect patients to necessary resources for improved well-being

Required qualifications

• 3 years of experience in healthcare coordination or community health work
• Experience with a team-based approach to care
• Excellent knowledge of community resources
• Intermediate skills with MS Office Suite, including Outlook and Teams
• Ability to work effectively in a remote environment with necessary internet speeds
