Care Coordinator Specialist

Job Summary

A company is looking for a Care Coordinator - Platform Specialist (Remote).

Key Responsibilities

• Collaborate with clinical leadership to develop and document client-specific clinical workflows
• Serve as the primary point of contact for on-service providers and address provider pain points
• Partner with internal teams to launch new programs and ensure alignment with organizational goals

Required Qualifications

• Bachelor's degree in a relevant field
• 1-2 years of experience in healthcare services, case management, or hospital administration
• 2+ years of experience in EMR training or clinical systems support
• Field-related certifications are a plus
• Familiarity with common EMRs used in behavioral health
