Care Coordinator - Wood County

Job Summary

A company is looking for a Care Coordinator who is responsible for comprehensive, multi-system care coordination activities for youth with complex needs and their families.

Key Responsibilities

• Engage children/youth with significant behavioral health needs and their families for assigned caseloads
• Link youth and families to community resources and conduct comprehensive assessments to develop individualized care plans
• Facilitate Child and Family Team meetings, ensuring the child and family have a voice in their care and access to necessary services

Required Qualifications

• At least 3 years of experience in community-based services for children and youth, or relevant education with reduced experience
• Experience in family systems, case management, or related fields
• Culturally competent with training to manage complex cases involving children and families
• Must complete high-fidelity wraparound training and hold valid certifications within specified timeframes
• Valid driver's license and acceptable driving record required
