Job Summary

A company is looking for a Care Facilitator.

Key Responsibilities

• Respond to patient and family inquiries and escalate urgent triage calls to the clinical team
• Coordinate and schedule appointments and referrals for follow-up care
• Monitor patients' progress and communicate any changes or health risks to clinical leadership

Required Qualifications

• Some college or an associate degree in clinical sciences, Nursing, or a related field
• Minimum two years of experience in collecting referral information in the healthcare market
• Medical Assistant certification preferred but not required
• Experience with electronic medical record software and proficiency in Microsoft Office
• Ability to multitask and manage multiple projects simultaneously
