Job Summary

As a vital member of the integrated care team, the full-time Care Guide will provide comprehensive support services to members, managing patient communications, coordinating care, and ensuring operational efficiency in a remote environment.

Key responsibilities
  • Manage in-app patient communication and initiate proactive outreach to encourage participation in health programs
  • Coordinate patient journeys by scheduling appointments, managing medication prior authorizations, and preparing essential forms
  • Ensure compliance with HIPAA guidelines while handling administrative tasks such as fax management and medical record retrieval
Required qualifications
  • At least 1 year of experience in a healthcare support, administrative, or customer service role
  • Proficiency with new technologies and digital systems
  • Strong organizational abilities and a proactive mindset for issue resolution
