Care Guide Representative

Job Summary

A company is looking for a Care Guide Representative.

Key Responsibilities

• Acts as the central facilitator to patients and supports them through all stages of care
• Identifies and resolves barriers to patient care and ensures connections to necessary resources
• Develops patient-driven goals and utilizes behavior change techniques to empower patient participation in their health

Required Qualifications

• Must be 18 years of age with relevant education and/or experience
• 0 to 2 years of experience in customer service, health care, or community-based role
• High school diploma or GED preferred
• Bachelor's degree in health care is a plus
