Care Management Associate

Job Summary

A company is looking for a Care Management Associate - Remote.

Key Responsibilities

• Support comprehensive coordination of medical services including Care Team intake and screening
• Assist in the implementation of care plans to promote effective utilization of healthcare services
• Promote and support the quality effectiveness of healthcare services

Required Qualifications

• 2-4 years' experience as a medical assistant or office assistant
• High school diploma or 5+ years equivalent experience
• Computer literacy, including proficiency in Excel and Microsoft Word
• Familiarity with basic medical terminology and concepts used in care management
• Strong customer service skills for coordinating service delivery
