Key Responsibilities

• Conduct telephonic outreach to engage and enroll eligible members into care management
• Facilitate case assignment and connect members to care managers through warm transfers
• Provide support services to team members and maintain accurate documentation of member interactions

Required Qualifications

• 2-4 years of experience in the healthcare field (e.g., medical office, hospital setting)
• Ability to meet daily metrics with speed and accuracy
• Strong customer service skills with a focus on positive outcomes for members
• High school diploma, G.E.D. or equivalent
• Experience with documentation in electronic records
