Job Summary

A company is looking for a Care Management Associate for its Engagement Hub, working from home.

Key Responsibilities

• Conduct telephonic outreach to engage and enroll eligible Medicaid members
• Facilitate case assignments and connect members to care managers through warm transfers
• Maintain accurate documentation of member interactions to ensure compliance with regulations

Required Qualifications

• 2-4 years of experience in the healthcare field (e.g., medical office, hospital setting)
• Ability to meet daily metrics with speed and accuracy
• Strong customer service skills with a focus on proactive issue resolution
• Experience in completing detailed documentation in electronic records
• High School diploma, G.E.D. or equivalent required
