Job Summary

A company is looking for a Care Management Associate for the Engagement Hub, working from home.

Key Responsibilities

• Conduct telephonic outreach to engage and enroll eligible members in healthcare services
• Facilitate case assignment and connect members to care managers through warm transfers
• Maintain accurate documentation of member interactions to ensure compliance with regulatory standards

Required Qualifications

• 2-4 years of experience in the healthcare field, such as in a medical office or hospital setting
• Ability to meet daily metrics with speed, accuracy, and a positive attitude
• Strong customer service skills to coordinate service delivery and resolve issues proactively
• High school diploma, G.E.D., or equivalent education
• Experience working independently while meeting deliverables and deadlines
