A company is looking for a Care Management Associate.

Key Responsibilities

• Conduct telephonic outreach to Medicaid members for enrollment and care coordination
• Facilitate case assignments and connect members to care managers through warm transfers
• Provide support to team members by answering calls, researching information, and maintaining documentation

Required Qualifications

• 2 years of experience in the healthcare field (e.g., medical office, hospital setting)
• Proficiency in Microsoft Word, Outlook, and Excel for data entry and documentation
• Familiarity with basic medical terminology and care management concepts
• Flexibility to work occasional nights and weekends
• High School diploma or equivalent GED
