Job Summary

Key Responsibilities

• Conduct telephonic outreach and enrollment of eligible Medicaid members
• Facilitate case assignments and connect members to care managers through warm transfers
• Provide support services to team members, ensuring compliance with policies and maintaining accurate documentation

Required Qualifications

• 2 years' experience in the healthcare field (e.g., medical office, hospital setting)
• Proficiency in Microsoft Word, Outlook, and Excel for data entry and documentation
• Familiarity with basic medical terminology and care management concepts
• Flexibility to work occasional nights and weekends
• High School diploma or equivalent GED
