Job Summary

A company is looking for a Care Management Associate to support the coordination of medical services and care plans.

Key Responsibilities

• Support comprehensive coordination of medical services including Care Team intake and screening
• Assist in the implementation of care plans to promote effective utilization of healthcare services
• Promote and support the quality effectiveness of healthcare services

Required Qualifications

• 2-4 years' experience as a medical assistant or office assistant
• High school diploma or 5+ years equivalent experience
• Computer literacy to navigate internal/external computer systems, including Excel and Microsoft Word
