Care Management Associate

Job Summary

A company is looking for a Care Management Associate - Remote.

Key Responsibilities

• Support coordination of medical services, including intake, outreach, and care plan implementation
• Track Medicaid redetermination and research eligibility issues
• Assist Case Managers with coordination of benefits and maintain accurate documentation

Required Qualifications

• 2-4 years of experience as a medical assistant or office assistant
• Strong customer service skills to promote positive outcomes for members
• Computer literacy, including proficiency in Excel and Microsoft Word
• Ability to effectively participate in a multi-disciplinary team
• Bachelor's degree in health or human services field preferred
