Job Summary

Supporting comprehensive healthcare coordination, the full-time Care Management Associate will engage telephonically with Medicaid members for outreach and enrollment, facilitating case assignments and care management connections while working remotely.

Key responsibilities

• Conduct telephonic outreach to Medicaid members, promoting enrollment into care management services
• Facilitate warm transfers to care managers and assist in implementing care plans
• Maintain accurate documentation of member interactions to ensure compliance with regulatory standards

Required qualifications

• 2-4 years of experience in the healthcare field, such as in a medical office or hospital setting
• Ability to meet daily performance metrics with speed and accuracy
• Strong customer service skills and proactive problem-solving abilities
• Experience with electronic record documentation and compliance requirements
• High school diploma, G.E.D., or equivalent education
