Care Management Associate

Job Summary

Supporting comprehensive healthcare coordination, the full-time Care Management Associate will engage telephonically with Medicaid members, facilitate case assignments, and ensure effective utilization of healthcare services while working remotely.

Key responsibilities

• Conduct telephonic outreach to enroll eligible members and connect them with care managers
• Provide support services to team members, including answering calls and maintaining accurate documentation
• Adhere to compliance standards and protect the confidentiality of member information

Required qualifications

• 2 years of experience in the healthcare field, such as in a medical office or hospital setting
• Proficiency in Microsoft Word, Outlook, and Excel for data entry and documentation
• Familiarity with basic medical terminology and care management concepts
• Flexibility to work occasional nights and weekends outside standard business hours
• High school diploma or equivalent GED
