Job Summary
A company is looking for a Care Management Support Coordinator II.
Key Responsibilities
- Perform outreach to members and support care plan next steps, resources, and scheduling
- Connect members to health plan and community resources to ensure high-quality customer care
- Document and maintain non-clinical member records in compliance with state and regulatory requirements
Required Qualifications
- High School diploma or GED
- 1 - 2 years of related experience
- Knowledge of assigned health plan activities and resources
- Understanding of Social Determinants of Health (SDOH) needs
- For Florida-Sunshine Health Plan, all interactions with members are done telephonically
