Job Summary

A company is looking for a Care Management Support Coordinator II.

Key Responsibilities
  • Provides outreach to members to support care plan next steps and connect them with health plan resources
  • Serves as front-line support for member and provider inquiries, explaining care plan procedures and protocols
  • Documents and maintains non-clinical member records in accordance with state and regulatory requirements
Required Qualifications
  • High School diploma or GED
  • 1 - 2 years of related experience, preferably in Healthcare Call Center, Customer Service, or Claims
  • Knowledge of assigned health plan activities and resources
  • Ability to connect members with community resources addressing Social Determinants of Health (SDOH)
  • For Florida-Sunshine Health Plan, all interactions with members must be conducted telephonically
