Job Summary
A company is looking for a Care Management Support Coordinator II.
Key Responsibilities
- Provides outreach to members to support care plan next steps and connect them with health plan resources
- Serves as front-line support for member and provider inquiries, explaining care plan procedures and protocols
- Documents and maintains non-clinical member records in accordance with state and regulatory requirements
Required Qualifications
- High School diploma or GED
- 1 - 2 years of related experience, preferably in Healthcare Call Center, Customer Service, or Claims
- Knowledge of assigned health plan activities and resources
- Ability to connect members with community resources addressing Social Determinants of Health (SDOH)
- For Florida-Sunshine Health Plan, all interactions with members must be conducted telephonically
