Care Management Enrollment Specialist

Job Summary

Working remotely or in-office in Richmond or Lynchburg, Virginia, the full-time Associate Care & Wellness Program Enrollment Specialist will provide person-centered care management services to long-term care insurance policyholders, conducting assessments and influencing participation in wellness programs.

Key responsibilities

• Respond to inbound inquiries and conduct outbound calls to educate policyholders and their families about the program
• Perform comprehensive assessments to identify key risks and recommend appropriate interventions for aging in place
• Develop rapport with policyholders and their care teams, providing education on aging resources and facilitating next steps in the care process

Required qualifications

• 2-5 years of professional experience working with older adults and their families in various care settings
• College degree or certification in a related field such as gerontology, nursing, or social work
• Experience in program enrollment or assessments geared towards seniors
• Strong written and verbal communication skills with a focus on customer service
• Ability to work independently and within a team, managing multiple responsibilities effectively
