Job Summary

Leading the advancement of care management programs, the full-time Manager, Care Management will oversee initiatives, ensure compliance, and foster collaboration while working remotely or onsite in Madison or Onalaska, WI.

Key responsibilities

• Oversee and enhance Care Management programs, services, and workflows in partnership with leadership
• Serve as a representative of the Care Management department in internal and external engagements, promoting visibility and collaboration
• Direct talent management activities, ensuring staff development, performance management, and adherence to organizational policies

Required qualifications

• Bachelor's degree in social sciences, health education, nursing, or a related field with 5+ years of healthcare experience
• 1+ year of formal people-leadership experience and 3+ years of case management experience
• Proven ability to develop and manage initiatives that deliver results while fostering teamwork
• Recognized expertise in care management across clinical, social, behavioral, and population health domains
• Strong understanding of government and regulatory requirements related to care management
