Care Management Outreach Specialist

Job Summary

A company is looking for a Care Management Outreach Specialist, responsible for scheduling members for enrollment into a care management program.

Key Responsibilities

• Make outbound enrollment calls to members using an automated dialer, meeting productivity and quality metrics
• Explain the VBCM program and schedule appointments between members and Care Managers
• Collect, input, and update member data accurately and provide clerical support as needed

Required Qualifications

• High school diploma required
• Minimum of one year experience in a sales-driven, call center, and/or customer service environment
• Experience in clinical environments preferred
