Job Summary

A company is looking for a Care Management Resource Center Specialist who will support patient care transitions remotely.

Key Responsibilities

• Support the Transitional Care Coordinator and Inpatient Social Worker with non-clinical tasks
• Initiate contact with payers and post-acute agencies to facilitate patient transfers and manage documentation
• Track essential data and assist in the collection of routine statistics related to patient discharge plans

Required Qualifications

• High School Diploma or GED
• Three to five years of administrative/clerical experience in healthcare or a related field
• Fluency in medical terminology
• Computer literacy
