Job Summary

A company is looking for a Care Management Support Coordinator II.

Key Responsibilities
  • Provides outreach to members to support care plan next steps and connect them to health plan and community resources
  • Serves as the front-line support for member and provider inquiries, explaining care plan procedures and protocols
  • Documents and maintains non-clinical member records in accordance with state and regulatory requirements
Required Qualifications
  • High School diploma or GED
  • 1 - 2 years of related experience in healthcare, managed care, or a medical/call center environment
  • Knowledge of health plan activities and resources
  • Experience with administrative care management activities
  • Understanding of Social Determinants of Health (SDOH) needs
