Job Summary
Supporting a clinical NICU team, the full-time Care Management Support Coordinator will manage administrative care activities, including outreach, inbound calls, and service scheduling in a remote setting.
Key responsibilities
- Perform outreach to members to assist with care plans, resources, and scheduling inquiries
- Provide support to connect members with health plan and community resources to ensure quality care
- Document and maintain non-clinical member records in compliance with state and regulatory requirements
Required qualifications
- High School diploma or GED
- 2 - 4 years of related experience in care management or clinical support
- In-depth knowledge of care management support activities and community resources
- Experience in member onboarding and administrative duties
- Ability to handle member and provider inquiries effectively
