Job Summary
Supporting a clinical NICU team, the full-time Care Management Support Coordinator will perform outreach, answer inbound calls, and schedule services while working remotely.
Key responsibilities
- Provide outreach to members to assist with care plan next steps and connect them to health plan and community resources
- Serve as the front-line support for member and provider inquiries, explaining care plan procedures and protocols
- Document and maintain non-clinical member records in compliance with state and regulatory requirements
Required qualifications
- High School diploma or GED
- 2 to 4 years of related experience in care management support
- Experience working with clinical NICU teams
- In-depth knowledge of care management activities and community resources
- Ability to support member onboarding and administrative duties
