Job Summary
Supporting clients facing various challenges, the fully remote Care Manager will research, assess, and plan interventions while facilitating access to community resources for long-term care insurance policyholders.
Key responsibilities
- Communicate with clients and families to identify barriers to aging in place and reduce out-of-pocket expenses
- Utilize motivational interviewing techniques to engage individuals with diverse health needs
- Document communication and intervention activities accurately and consistently
Required qualifications
- Strong background in geriatric case management or coordination experience
- Experience in Social Services, Care Coordination, Healthcare Navigation, or related fields preferred
- 1 year of relevant experience in a customer service or similar role preferred
- Bachelor's degree in Psychology or Social Work preferred
- Must be 18 years of age or older
