Care Manager

Job Summary

A company is looking for a Care Manager.

Key Responsibilities:

• Provide case management and support for members in need of behavioral health care
• Coordinate with various departments, health plans, facilities, and providers to ensure quality care
• Engage with members, develop care plans, and facilitate care transitions for optimal health outcomes

Required Qualifications:

• Current, unrestricted state license to practice independently as a Clinical Social Worker, Marriage and Family Therapist, Professional Counselor, Clinical Psychologist, or Registered Nurse
• Minimum of 2 years of direct clinical care experience
• Strong computer skills and ability to document information while on the phone
• Certified Case Managers (CCM) preferred with requirement to obtain CCM within the first three years of hire
• Interest in professional development through specialization, certification, and/or advanced degree
