Care Manager - Health Plan - Remote

Job Summary

A company is looking for a Care Manager (BH) for a Health Plan position.

Key Responsibilities:

• Responsible for assessment, care planning, and coordination of care for high and moderate members with behavioral health conditions
• Develops individualized care plans and conducts health risk assessments
• Advocates for members' care needs and monitors effectiveness of interventions

Qualifications:

• Current licensure as LMSW, LCSW, LMFT, LPC, or RN
• Minimum 5 years' experience in healthcare, behavioral health, or substance abuse setting
• Knowledge of utilization management procedures and DSM-5 diagnostic edition
• Bachelor's in Nursing or Master's in Counseling/Social Work required
• Strong organization and time management skills
