Key Responsibilities:

Develop, assess, and adjust care plans to promote desired outcomes
Coordinate services between healthcare providers to meet clients' needs
Provide patient and provider education, facilitate access to community services, and monitor referrals

Required Qualifications:

Graduate from an Accredited School of Nursing
2+ years of clinical nursing experience in various settings
Bachelor's degree in Nursing preferred
Current state RN license and clinical licensure for multiple states
Knowledge of healthcare and managed care preferred
