A company is looking for a Care Manager.

Key Responsibilities:

• Provide care management services to a panel of patients
• Educate patients on chronic conditions, self-management skills, and goal setting
• Conduct Transitional Care Management activities for high-risk patients to reduce readmissions

Required Qualifications:

• Licensed Practical Nurse required
• 1+ years of experience in healthcare serving chronically ill patients
• Experience in telephonic case management or care coordination
• Knowledge of medical terminology
• Experience working in a care team environment
