Job Summary

A company is looking for a Care Navigation Guide to join their Care Navigation team.

Key Responsibilities:

• Outreach to providers and facilities to provide personalized care options for members
• Collaborate with RNs on complex cases to effectively route care
• Communicate trends to leadership to improve processes and member experience

Required Qualifications:

• 1+ years of experience in care coordination and navigation in a healthcare setting
• 1+ years of experience utilizing a Chromebook and Google Workspace
• Ability to confirm and update provider data
• Experience with compliance in applicable laws and regulations
• Proficiency in documenting tickets and staying current on processes and workflows
