Job Summary

Leading a high-performing Patient Care Specialist team, the full-time remote Care Navigation Manager will oversee daily operations, ensure efficient patient case management, and act as a senior escalation point for complex situations while fostering a culture of empathy and continuous improvement.

Key responsibilities
  • Lead, coach, and develop a team to deliver exceptional patient care experiences
  • Establish and monitor performance metrics to ensure patient satisfaction and operational efficiency

Required qualifications
  • 5+ years of experience in patient navigation, care coordination, or digital health operations
  • 3-5 years of people management experience in healthcare or customer support environments
  • Strong organizational and data-driven skills for monitoring performance metrics
  • Demonstrated ability to handle sensitive patient information with discretion and HIPAA compliance
  • Experience with self-funded employer benefit plans or bundled payment models is a plus
