Care Navigation Program Manager

Job Summary

Overseeing the Care Navigation program, the full-time remote Care Navigation Program Manager will manage health plan partner relationships, ensure efficient program operations, and provide telephonic care navigation services to individuals during implementation and onboarding periods.

Key responsibilities

• Provide telephonic care navigation services, including screening, navigation, and care management support
• Create and maintain clinical SOPs and workflows that are evidence-based and adhere to standards of care
• Supervise a team of frontline care navigators, assigning and monitoring their daily and weekly work activities

Required qualifications

• Experience in care navigation or related healthcare field
• Proficiency in using technology platforms for care management
• Ability to develop and implement clinical SOPs and workflows
• Experience in supervising and managing teams
• Knowledge of industry best practices in care navigation services
