Care Navigation Program Manager

Job Summary

Overseeing the Care Navigation program, the full-time remote Care Navigation Program Manager will manage relationships with health plan partners, ensure efficient operation of existing programs, and provide telephonic care navigation services to individuals during onboarding and implementation periods.

Key responsibilities

• Provide telephonic care navigation services and support to individuals through the New York Social Care Network
• Create and maintain clinical SOPs and workflows that meet standards of care, while documenting all activities accurately
• Supervise a team of frontline care navigators, assigning and monitoring their daily and weekly work activities

Required qualifications

• Experience in care navigation or a related healthcare field
• Knowledge of care management systems, such as Unite Us
• Ability to create and maintain clinical standard operating procedures
• Experience supervising teams in a healthcare setting
• Willingness to travel occasionally for orientation and team meetings
