Job Summary

A company is looking for a Care Navigation Guide to join their Care Navigation team.

Key Responsibilities

• Engage with providers to curate personalized care options for members while adhering to service level agreements
• Collaborate with RNs on complex cases to ensure effective care routing
• Identify and communicate trends to leadership to enhance processes and member experience

Required Qualifications

• 1+ years of experience in care coordination and navigation within a healthcare setting
• 1+ years of experience using a Chromebook and Google Workspace
• Ability to confirm and update provider data accurately
• Experience in documenting tickets and staying current on workflows
• Compliance knowledge with applicable laws and regulations
