Care Navigation Specialist

Job Summary

To enhance member care experiences, the remote Care Navigation Guide will curate provider lists, engage with network providers, and collaborate with RNs on complex cases while residing in Arizona, Florida, Georgia, or Texas.

Key responsibilities

• Outreach to providers and facilities to deliver personalized care options that meet members' requests and access standards
• Collaborate with RNs on complex cases to ensure effective routing of care
• Document provider data accurately and communicate trends to leadership for process improvement

Required qualifications

• 1+ years of experience with care coordination and navigation in a healthcare setting
• 1+ years of experience utilizing a Chromebook and Google Workspace
