Job Summary
Supporting patients and caregivers in a full-time remote capacity, the Care Navigation Specialist will provide compassionate outreach, resource navigation, and essential clinical operations assistance while managing administrative workflows and documentation follow-up.
Key responsibilities
- Serve as a point of contact for patients and family caregivers, connecting them with appropriate services and community resources
- Support appointment scheduling, new patient intake, and maintain accurate patient information for clinicians
- Document patient interactions and assist with identifying and following up on missing documentation and outstanding tasks
Required qualifications
- Bachelor's degree in Social Work (BSW) or a related human-services field
- Experience supporting patients, caregivers, or individuals navigating health and social needs through employment, internship, or practicum
- Foundational knowledge of community resources, healthcare navigation, and caregiver support
- Ability to recognize when escalation to a clinician or supervisor is necessary
- Comfortable working in a fast-paced healthcare environment and learning new technology and workflows
