Job Summary

A company is looking for a Care Navigator.

Key Responsibilities:

• Check in with patients regularly to assess care needs and provide disease education and coordination
• Schedule patients with interdisciplinary care team members as needed
• Facilitate care coordination with other medical providers and arrange necessary tests or referrals

Required Qualifications:

• Passion for helping patients with disabilities
• Experience in a healthcare environment preferred
• Experience delivering persuasive messaging and empathy
• Ability to work in a metric-driven and goal-oriented environment
• Experience with Thrio and Elation systems preferred
