Key Responsibilities

• Guide dyads through the dementia care journey by providing support, education, and resources
• Conduct comprehensive assessments to understand dyad needs and develop personalized care plans
• Facilitate communication between dyads, the care team, and healthcare providers, while maintaining accurate records

Required Qualifications

• Associate's or Bachelor's degree required; relevant experience may substitute for formal education
• Minimum of 2 years of professional experience with the geriatric population
• Prior experience with dementia or cognitive impairments preferred
• Ability to work in a virtual environment with a private and quiet workspace
• Strong organizational skills and proficiency in maintaining records
