Care Navigator

Job Summary

A company is looking for a Care Navigator for the East Coast.

Key Responsibilities

• Process referrals to secondary care and maintain directories of specialists and community services
• Manage communication and documentation for referrals to ensure continuity of care
• Serve as the primary contact between patients, specialists, and clinic physicians regarding external care needs

Required Qualifications

• At least 3 years of applicable experience, preferably in a Primary Care setting
• High school diploma or GED required
• Proficient in Google spreadsheets and/or Excel, with experience in database management and data tracking
